Providence is calling a Licensed Clinical Professional Counselor (Behavioral Health, Day Shift, 1.0 FTE) to Providence St. Joseph Medical Center in Polson, Montana. In this position you will:

  • Assesses patient/family learning needs, abilities, preferences and readiness to learn. Completes on going assessments and documents according to policy. Communicates with other members of the health care team regarding the patient’s needs, plan and response to care. Evaluates response to plan and makes adjustments as indicated.

  • Assesses, collaborates and coordinates patient’s discharge planning needs and readiness for discharge with other members of the care team.

  • Consults regarding cases that are presenting problems and/or experiencing significant deviations from the plan of care, conferring with the Medical Director/Department Director as needed.

  • For the ages listed, screens, assesses, monitors, documents and interprets psychosocial, spiritual, environmental, self-care, educational, and discharge planning needs.

  • In collaboration with the patient/family, and in a developmentally and culturally appropriate manner, implements the plan of care according to established Social Work policy.

  • Maintains current knowledge of reimbursement and regulatory issues of the payors most frequently seen with the patient population.

  • Obtains a history through the interview process, maintains ongoing monitoring/assessment of assigned patients.

  • Participates in the development of patient plan of care to include patient problems, goals, evaluations and interventions.

  • Reports symptoms and significant changes in patient’s condition to appropriate personnel.

  • Utilizes appropriate teaching methods and/or referrals to ensure patient/family educational needs are met. Provides community resource information to patient/family and evaluates the effectiveness of teaching, making adjustments as necessary. Documents education process on appropriate screen/format.

  • Works with health care team and outside resources to facilitate optimum patient care delivery.

  • Educates health team colleagues about Social Work. Including the role and unique needs of the case-managed patient populations and serves as a resource for physicians and other departments.

Qualifications:

Required qualifications for this position include:

  • Master's Degree in Professional Counseling

  • Montana Licensed Professional counselor is required upon hire. Non Violent Crisis Intervention is required within 6 months of hire with recertified every 2 years after

  • 3 years experience with adult and/or adolescent populations and familiarity with broad range of psychiatric disorders

  • Well developed clinical skills for interviewing and assessing
