Providence Health & Services Pharmacist (Temporary) in Polson, Montana
Providence is calling a temporary Pharmacist (Pharmacy, Day Shift, 1.0) to Providence St. Joseph Medical Center in Polson, Montana. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
In this position you will:
Fill medication orders to all inpatient and/or outpatient areas.
Provide pharmacy anticoagulation services in an outpatient setting.
Individualize anticoagulation therapy based on indications, concurrent disease states, lab data, concurrent drug treatment, and pharmacokinetics.
Provide patient-focused care by recommending, monitoring, and evaluating patient-specific pharmacotherapy.
Interview to ensure appropriate patient history data is documented.
Gather information via chart review, patient interviews, and patient's retail pharmacy.
Provide quality patient specific drug information independently by utilizing appropriate resources.
Ensure prompt and accurate delivery of medications to patient, while maintaining confidentiality.
Review, process and check physician orders and patient medication profiles.
Provide pharmaceutical and drug interaction information to the medical staff, hospital staff, and the public.
Have responsibility for recommending changes in a patient's drug therapy and capable in interpreting and evaluating physician orders for the most cost effective, appropriate therapy.
Ensure the consistency of quality and standards of practice.
Required qualifications for this position include:
- Current license or eligible for license as a Registered Pharmacist in Montana State
Preferred qualifications for this position include:
Bachelor of Science from an accredited School of Pharmacy
Doctorate of Pharmacy (PharmD)
Prior hospital pharmacy experience
