Providence Health & Services Pharmacist in Polson, Montana
Providence is calling a Pharmacist (Lakeshore Pharmacy, Variable Shift, 1.0 FTE) to Providence St. Joseph Medical Center in Polson, Montana. In this position you will:
Check for appropriate therapeutic use, appropriate dosage, duplicates, allergies, drug interactions, order changes and contraindications.
Contact MD if data suggest sub- or supra- Therapeutic drug therapy or if patient safety is at risk.
Prepare pharmacokinetic consult if appropriate.
Contact MD or nurse as necessary to clarify orders, resolve problems identified during review.
Demonstrate the ability to assess patients medication therapy needs based on diagnosis, lab values, medical history, and current physical status.
Determine identity, dose and administration schedule of medication or IV admixture ordered.
Document intervention and its outcome for use in drug therapy quality improvement efforts.
Ensure that dosage form is properly labeled. Attaches any auxiliary labels that are required.
Ensure that inpatient medication gets to the inpatient by appropriate transport mechanism.
Oversee technician support of the dispensing process and checks work before prescription goes to a patient.
Perform calculations necessary for dosage form preparation and compounds the dosage forms.
Accurately enter MD medication order into computer system.
Required qualification for this position include:
- Licensed to practice pharmacy in the state of Montana
Preferred qualifications for this position include:
Doctorate of Pharmacy (PharmD) or Bachelor's Degree in Pharmacy
Prior hospital pharmacy and outpatient anticoagulation experience
